On August 2, 1971, American Apollo 15 mission commander David Scott placed a poignant memorial on the surface of the moon. Designed by Belgian artist Paul von Hoeydonck the memorial, titled Fallen Astronaut, comprised an aluminium statuette depicting a space-suited astronaut and a small plaque bearing the names of 14 astronauts and cosmonauts who had died in the line of duty. Among those listed were the crew of Apollo 1 - Gus Grissom, Ed White, and Roger Chaffee - who perished in a launch pad fire on January 27, 1967; American astronaut trainees Theodore Freeman, Charles Bassett, Elliott See, Edward Givens, and Clifton Williams, who died in aircraft and automobile accidents before they could fly into space; Soviet cosmonaut Vladimir Komarov, who died during the disastrous Soyuz 1 mission on April 24, 1967; and the first man in space, Yuri Gagarin, who died in a mysterious plane crash the following year. Ever since Gagarin made his historic orbit of the earth on April 12, 1961, humanity has logged over 55,000 person-hours in space. But spaceflight has always been a dangerous business, and in the process of exploring the cosmos we have added many more names to the memorial roll, including the fourteen crew members lost in the January 28, 1986 Challenger and February 1, 2003 Columbia space shuttle disasters. Yet of all these martyrs to the advancement of human knowledge, nearly all have perished within the confines earth’s atmosphere. But nearly isn’t all, so who are the humans who’ve died in space, and how did it happen?
