During an international medicine trip to Honduras this summer, I performed a paraspinous cervical injection with 0.5% bupivacaine to treat a 76-year-old female with a severe headache for 2 months. The neurologic examination was unremarkable, and the history was not consistent with a subarachnoid bleed or cerebral venous thrombosis. Her vital signs were normal except for a slight increase in blood pressure. We treated her headache using a technique my twin brother discovered, and I first reported on approximately two decades ago. She had complete headache relief in under five minutes.
